Medicare Part A: A Simple, Clear Overview

Medicare can feel like a maze, especially when you’re trying to understand what each part actually covers. Medicare Part A-often called hospital insurance-is one of the core components of Original Medicare, and it plays a major role in protecting your finances during serious medical events.

Medicare Part A is the part of Medicare that helps pay for your care when you’re admitted to a hospital or similar facility. Think of it as your safety net for major medical situations-hospital stays, skilled nursing care after an illness or surgery, hospice care, and some home health services.

Most people don’t pay a monthly premium for Part A because they (or their spouse) paid Medicare taxes while working. That’s why it’s often referred to as “premium-free Part A.”

What Part A Covers

1. Inpatient Hospital Stays- If you’re formally admitted to a hospital:

  • Semi-private rooms
  • Nursing care
  • Meals
  • Medications during your stay
  • Operating and recovery room services

It doesn’t cover private rooms (unless medically necessary) or personal items.

2. Skilled Nursing Facility (SNF) Care- This is short-term rehab after a qualifying hospital stay (not long-term custodial care):

  • Skilled nursing
  • Physical, occupational, or speech therapy
  • Medical supplies and medications

3. Hospice Care- For people with a terminal illness. Part A covers comfort-focused care, including:

  • Pain and symptom management
  • Nursing services
  • Counseling
  • Certain medications

4. Limited Home Health Care- May cover part-time skilled nursing or therapy at home when medically necessary.

What Part A Doesn’t Cover

  • Long-term custodial care
  • Private-duty nursing
  • Doctor visits during your hospital stay (these fall under Part B)
  • Personal comfort items

What It Costs

Even if you don’t pay a premium, you still have cost-sharing:

  • Deductible: You pay $1,736 for each benefit period (not yearly) for 2026
  • Hospital coinsurance:
    • Days 1–60: $0 after the deductible is met
    • Days 61–90: $434/day copay
    • 91-150: $868/day copay (using lifetime reserve days)
    • After day 150: You pay all costs
  • Skilled nursing coinsurance:
    • Days 1–20: $0
    • Days 21–100: $217/day copay

Hospice care usually has minimal out-of-pocket costs.

How It Works in Real Life

If you’re admitted into the hospital, you pay the Part A deductible, and Medicare covers the rest of your inpatient costs for that benefit period from days 1-60. You are responsible for the per day copay if you stay beyond 60 days. If you need rehab after being discharged, and qualify for skilled nursing care, the first 20 days are fully covered, then you will pay the per day copay for days 21-100.

Who’s Eligible

You qualify for Part A at age 65 if you’re a U.S. citizen or permanent resident and you or your spouse worked and paid Medicare taxes for at least 10 years. Some people qualify earlier due to disability or certain medical conditions.
